IN-NETWORK MENTAL HEALTH INSURANCE

We are in network with Blue Cross Blue Shield, MODA, and PacificSource commercial plans.

We will ONLY bill in-network insurance. It is your responsibility to check your insurance benefits! We strongly encourage you to call your insurance company to confirm your eligibility and benefits for the specific service codes that we will bill. We will provide you with the necessary information to do so.

If we are in network with your insurer, we will bill your insurance first and your account will be billed once we have received the insurance payment. This can sometimes take several weeks.

OUT-OF-NETWORK MENTAL HEALTH INSURANCE

If you would like to use your insurance but we are not in-network (preferred provider) with your insurance carrier, it is your responsibility to submit your cost for reimbursement. We will provide you with a 'super bill" that you will need for reimbursement purposes.

BILLING AND PAYMENTS FOR PSYCHOLOGICAL SERVICES WITHOUT INSURANCE

If you pay for your service out of pocket (without billing insurance), we will provide you with an estimate of cost prior to the service. Payment for a psychological evaluation (testing) will be due in three installments. You will pay after your intake session, after the psychological testing, and before the full report will be released. Payment plans can be arranged.

CANCELLATION FEE

Cancellations 24 hours or less before the appointment time will be subject to a $100 cancellation fee.
